JOB SUMMARY:
Under the direction of the Clinical Manager, serves in clinical case management for assigned patient case load. Completes Psychosocial Assessments. Conducts physiotherapeutic groups, individual and family sessions and completes chart documentation in a timely manner.
QUALIFICATIONS:
Education: Master’s Degree in Social Work; Licensed Certified Social Worker (preferred)
Experience: Minimum of 1 year of psychiatric/chemical dependency in an acute/outpatient or residential setting.
Skills: Sound judgement, mature interpersonal skills, and leadership/management skills along with the ability for problem solving and strong conflict resolution skills. Ability to maintain a good working relationships and the skill to tolerate a high level of stress.
