JOB SUMMARY:
Counselor must have an understanding and be able to perform 12 core functions: patient screening, intake, orientation, assessment, treatment planning, counseling, case management and crisis intervention. The primary role of a counselor is to assist clients in reaching their optimal level of psychosocial functioning through resolving negative patterns, prevention, rehabilitation, and improving quality of life.
Duties will include adhering to maintaining a 1:12 caseload, assisting, monitoring or facilitating scheduled client activities. Duties also require maintaining client chart containing required documentation to maintain proficient client care under state legal and ethical guidelines and standards. All duties are inclusive and may include changes made by Program Manager and/or assigned designee.
